--Book here for a clinical Neuropsychological Assessment or Pre-Bariatric Surgery evaluation-- After you book, Dr. Emily will send you more information on how to prepare for the appointment, including how to complete pre-appointment information forms. Please check your "trash" for email from EpiphanyModernWholeHealth@outlook.com. Please allow 4-7 hours for the appointment. If needed, we can divide the time into two or more days. $950 paid at the time of booking is non-refundable at the time of booking. The remaining $950 is paid at the time of the first appointment (payment accepted via Square secure processing before the start of the appointment). Please call your insurance carrier if you’re wondering about reimbursement via a Superbill. Dr. Blegen has opted out of Medicare & Medicaid, which means you cannot submit to Medicare or Medicaid for reimbursement. This may be true for any other federal/government insurances, but you may check with your insurance to be sure. After 2: missed appointments / arriving after 30 minutes late / and/or rescheduling 14 days before the appointment - then the $950 of the $1900 fee will be consumed (and if the full $1900 was paid, then $950 will be refunded via Square). If an appointment that was started is not completed within 3 months of the first date booked: then $1425 of the fee will be consumed, $475 refunded via Square, and the evaluation cancelled. If a forensic or legal evaluation is booked (e.g., guardianship, college testing accommodation, because your lawyer told you to, testamentary capacity): $950 is non-refundable at the time of booking. If the an evaluation is discontinued for this reason, then $1425 of the fee will be consumed and $475 refunded via Square.
